ATHLETIC PARTICIPATION: CLEARANCE FORM & WELL EXAM
Well-Exam/Athletic Participation Clearance Forms are good for two years. Please download the form or call your doctor's office and have them send one directly to BAMS. Your student-athlete must have a doctor-signed document to participate.
